This would involve checking for intrinsic factor antibodies in suspected pernicious anaemia, examining for gastrointestinal disease of the ileum or gastric mucosa, and investigating any drug that could interfere with B12 absorption, such as metformin or proton pump inhibitors.

    A complete dietary history is important. Inquire about use of supplements, duration of being on a plant-based diet, and gastrointestinal symptoms. It is the integration of history, symptoms, and laboratory findings that provide a sound diagnosis.

    Treatment of B12 Deficiency

    Treatment varies according to the cause, severity of the condition, and how well the patient can absorb B12 by mouth.

    Parenteral therapy (injection) is most often the initial treatment when there are neurological symptoms or when absorption is definitely hampered. The usual regimen is to administer 1,000 micrograms of hydroxocobalamin by intramuscular injection a number of times early in the first few weeks and then maintenance doses two to three months apart.

    For less severe conditions or when absorption is normal, high-dose oral therapy is effective. While B12 absorption in the gastrointestinal tract is restricted by intrinsic factor, large oral doses (e.g. 1,000–2,000 micrograms per day) can still lead to adequate passive absorption. Oral therapy can be an option of choice for needle-phobic patients or for maintenance therapy when levels have normalized.

    Regular supplementation is often necessary. Through injection or oral administration, lifelong replacement is typical, particularly if dietary levels continue to be inadequate or the cause of impaired absorption is persistent.

    Optimum Practice for Clinics Working with Plant-Based Clients

    Clinics that regularly see vegans and vegetarians should be pro-active about B12 testing. Regular screening should be recommended for clients who are on strict plant-based diets, especially those who are not supplementing regularly.

    If a patient complains of fatigue, pins and needles, or cognitive change without explanation, B12 should be on the differential list. Lab work is easy and inexpensive compared to the risk of a missed diagnosis.

    Healthcare practitioners should give straightforward counsel regarding supplementation. Numerous patients incorrectly assume that a healthy vegan diet plan is nutritionally adequate by itself. It’s helpful to describe that plants don’t produce B12 and need to be derived from fortified foods or pills.

    Treatment always needs to be individualized. In the absence of impaired absorption, oral treatment can be adequate. In the presence of symptoms of gut disease or if there is any neurological component, begin with injections and reassess as the symptoms resolve.
